New Patient Registration

If you live within the practice boundary and would like to register with the practice, please collect a registration pack from the treatment room reception.

All newly registered patients need to book for a simple health check with the practice nurse who will carry this out. This gives us the opportunity to meet you and your family and tell you about the practice.

New patients have the right to express a preference of doctor on their records. However, the practice may not always be able to comply with such a request. In that event we would explian why. All patients may consult with any doctor working at the practice, who will have access to your medical records.
